Carrier Profile

We TORUS will be invoicing you every Friday but we make you sign this form just in case you are to busy and can’t take care of your invoice by Sunday, then we will be charging your credit card.

CREDIT CARD PAYMENT AUTHORIZATION FORM

I hereinafter called CARRIER do hereby authorize Torus transportation LLC, hereinafter called DISPATCH, to initiate a debit entry for the amount due on each invoice for services rendered per signed agreement, to the credit card account indicated below, in consideration of the dispatching service provided to me. I understand that my signature on this authorization form, along with a photocopy of the front and the back of both my credit card, as well as my driver license, will allow me the convenience of not having to produce these items for impression at the time of service.

I understand that I will be notified via email when DISPATCH debit my account each week.

This authorization is to remain in full force and effect until DISPATCH-CARRIER agreement is terminated.

I understand that if the load is tendered and accepted by me, but for any reason, whether is due to carrier, shipper, or broker, the load gets reschedule or cancelled, I am still responsible for paying DISPATCH as set out above unless agreement is made with Torus transportation, LLC. Any revocation shall not be effective until DISPATCH is notified by CARRIER in writing to cancel this automatic payment authorization, in such time and in such a manner as to afford DISPATCH a reasonable opportunity to act on it.

DISPATCHER & CARRIER AGREEMENT

This is the agreement between the dispatcher and the owner operator, fleet owner or truck driver.

I hereby grants authorization to TORUS TRANSPORTATION, LLC. to act as my agent for the sole purpose of searching for and booking shipments, processing all brokerage paperwork and obtaining

Certificates of Insurance as required in order to expedite shipments and dispatch via telephone, fax or e-mail for my truck, All billing, invoicing and collections of revenue from customers, brokers, shippers, consignees, etc- are the sole responsibility of the carrier. If revenue for a shipment or shipments are uncollectible, TORUS TRANSPORTATION, LLC will be held harmless and no penalty or deduction of fees will be made. The carrier agrees to maintain all proper licenses and permits to conduct business as a motor carrier in the area of intended operation.

Additionally, carrier agrees to maintain liability and cargo insurance at the amounts set forth by the home state of the carrier.

TORUS TRANSPORTATION, LLC will be held harmless in the event of any and all claims. The carrier agrees to maintain an account with (an internet load board service), in the name of the carrier, with

TORUS TRANSPORTATION, LLC. as the point of contact for dispatching purposes.

The fee for dispatch services will be 7 % of the gross revenue of each shipment with no minimum charge for the first 3 months and 10% there after.

As loads are picked up, an amount equal to the above stated percentage will be payable to: TORUS TRANSPORTATION, LLC. Payments are to be conveniently paid with any Debit or Credit Card via Text or Email Invoice App.

Either party has the right to end this agreement without cause at any time with seven (7) days notice by written request.

Statement of the Torus transportation will:

  1. Find freight that best matches profile for the Client.

2. Contact Client with load matches and go over options.

3. fax to shipper/broker the Client’s Authority, W-9, proof of insurance, and order insurance certificates if required, along with any other required supporting documentation upon the Client agreeing to take a load.

4. Handle the setting of appointments if necessary.

5. Provide the driver with all dispatch instructions for pickup, transit and delivery.

6. Assist with any problems that arise in the transit of the load when necessary if within our capabilities. The Client is responsible for own equipment. We can try to direct Client to a service that might be of help.

7. Hold on to the dispatch, accessorial information, etc. until the load is completed. Once completed Torus transportation will mail or fax all documents to the Client.

8. Forward the final load confirmation and mail all documentation to the Client, concluding that all services have been performed in full.

Consent *

I authorize TORUS TRANSPORTATION, LLC to complete all broker Carrier Packets and Rate Confirmations on my behalf LLC on my behalf.
